Customer Information Form

The business activity of ICOS S.p.A. is aimed exclusively at operators engaged in the resale of IT products.

To apply for registration of your company as a customer, you must complete the form below and upload your Company Registration Certificate issues not more than six months prior to the date of this application. Once you have completed and submitted the form, you will be asked to sign electronically the General Terms and Conditions.

Some information is mandatory so that we can properly manage business processes. Other will enable us to offer you the best support for the success of your business. So we ask you the courtesy of taking the time to provide us with as much information as possible.

PLEASE NOTE: Failure to complete this information sheet and sign the General Terms and Conditions will not allow ICOS to process any orders submitted.
