Thanks for submitting the Customer Information Form.

Now you’re just two steps away from completing your request: the signature of our General Terms and Conditions (GTC, click here for a preview), and the Export Compliance Awareness (ECA). Signing both documents is a necessary condition for ICOS to create your customer account. Signature collection is done electronically through the trustworthy DocuSign platform, which assures both you and us that the process is compliant. Please note: The person signing the documents must be a legal representative of the company, with signing authority.

Once signed the GTC and the ECA, you’ll shortly receive approval of your request, should the verification process be successful. In the meantime, if you have any needs you can contact us via the Information Request web form. Thanks for choosing us!
